You can have one provider manage everything, or put it together yourself from separate tests (see our guide). Every row is checked against the provider's own site and dated. By default providers are grouped by category, and no provider pays for its position.
Reading the filters: "Physician order: Included" means the provider says its own physician network orders the test, so you don't need your own doctor. "HSA / FSA" shows only what the provider itself says; your plan has the final say. The state filter hides providers that say they don't serve your state. Most providers don't publish a state list, so a provider still showing isn't a promise it's available where you live. Details in the FAQ.

How do you list providers?
Every price and fact on this page was checked against the provider's own US website on the date shown, not a search snippet or press release. Treat that date as an expiry, not a guarantee. Prices are in US dollars, shown the way the provider publishes them: where a price varies by state or location, we say "from". "Not stated" means we looked and the provider didn't publish it. A blank state list means the provider doesn't publish one, not that it's available everywhere. This page has no star ratings or written reviews of our own. No provider pays to be listed, and none can pay for a better position: the default order and every filter use only the published facts. Spotted something that's changed? What does "physician order" mean?
In most states, a lab test needs an order from a licensed clinician. Most direct-to-consumer providers handle this through their own physician network, so you don't need to see your own doctor first. On this page, "Included" means the provider says its network orders the test (Labcorp OnDemand, Quest (questhealth.com), Mito Health, Marek Health, Lifeforce, OneTwenty, BodySpec). "Not stated" means the provider didn't say on the pages we checked. None of the providers listed here say you need to bring an order from your own doctor. Checked Oct 9, 2026.
Can I pay with an HSA or FSA?
13 of the 22 providers here say on their own site that their test or membership is HSA/FSA eligible, sometimes with conditions ("most tests", "most members", "in most cases"): Function Health, Superpower, Everlywell, Labcorp OnDemand, Mito Health, Ultrahuman Blood Vision, SiPhox Health, Lifeforce, OneTwenty, DexaFit, Neko Health, TruDiagnostic, Tally Health. Ezra says its MRI "may be eligible" and to check with your plan administrator. The rest don't say. Eligibility depends on your own plan, so check with your plan administrator before you pay. Does health insurance cover these tests?
Mostly no: these are self-pay services. Biograph, Everlywell and Neko Health each say they don't accept insurance, and TruDiagnostic says its test isn't currently covered by insurance. The other providers don't say either way on the pages we checked. If a doctor orders a test as part of your regular care, your own insurance rules apply, and that's a question for your insurer. Are these tests available in every state?
Not always. Some states restrict direct-to-consumer lab testing, and providers handle that differently. Superpower is live in 41 states plus DC; the states not on its list are HI, ME, MT, NH, ND, SD, VT, WV and WY. Ultrahuman Blood Vision and OneTwenty both exclude Rhode Island, and both (along with Superpower) say New York and New Jersey can cost more. The other providers don't publish an exclusion list, which isn't the same as being available everywhere. In-clinic providers (Biograph, Human Longevity, Neko Health, Prenuvo, Ezra, BodySpec, DexaFit) depend on where their clinics are. What's the cheapest longevity blood test in the US?
It depends on how you count. Per test, SiPhox is $124 plus shipping, Ultrahuman Blood Vision starts from $99 every 6 months (the price varies by state), and Mito Health's Core panel is $197.59 plus a $9/mo membership. As an annual membership, Superpower is $349/yr (2 blood draws), Function Health $365/yr and Everlywell $399/yr. Which providers offer a bundled longevity program rather than a single test?
Two on this page. Biograph ($7,500 for the first year of its Core membership; Black is $17,500) combines whole-body MRI, DEXA, a blood panel and VO2 max with a physician-led review, in New York City and the San Francisco Bay Area. Human Longevity Inc. ($8,000/yr for Executive Health, for people under 65) combines genome sequencing, MRI, blood biomarkers and heart tests, in San Diego and San Francisco. Fountain Life offers a similar program but doesn't publish a price, so it isn't listed until it does. How much does a whole-body MRI cost in the US?
Ezra's Annual MRI is $999 (22 minutes; $899 for Function members), and Prenuvo's Whole Body scan is $2,499 (about 60 minutes; its Focused scan is $1,199). For body composition rather than MRI, a DEXA scan is much cheaper: BodySpec is $59.95, and DexaFit starts from $90 but varies by location. How do you handle FDA status and marketing claims?
We only state a test's FDA status where the provider or the FDA's own database says it, and we don't repeat a provider's marketing claims (about lowering your biological age, for example) as fact. A provider's own struck-through price isn't enough, because inflated former prices are exactly what the FTC's guides on deceptive pricing cover.
